Job Title : Traveling Home Health PT – $8,000 Retention Bonus + Flexible Autonomy

Company : Home Health Care, Inc.

Location : Twin Cities Metro area

Compensation : $100,000 - $200,000 annually + $8,000 Retention Bonus!

Your Day-to-Day :

  • Perform evaluations and develop home-based therapy plans
  • Collaborate with families, nurses, and care teams
  • Document care accurately and adjust plans as needed
  • Help clients achieve personal milestones through mobility and strength coaching

    • What You Need :

  • MN PT license in good standing
  • Solid documentation and communication skills
  • Driver’s license, vehicle, and current insurance
  • A desire to empower clients within their home environment
