POSITION SUMMARY

The Outreach Specialist will conduct community outreach at health centers, provider offices and other social service sites aimed at recruiting and enrolling eligible prenatal clients into the Welcome Baby program. They will work with various entities to obtain referrals, such as the OB clinics and providers with Labor and Delivery privileges. They will also be responsible for developing relationships with key community contacts, maintaining prenatal outreach logs, following up with and referring potential clients to the Welcome Baby program. The WB Outreach Specialist will work closely with the Welcome Baby Program Manager, Clinical Supervisor, Grant Coordinator/Hospital Liaison and other program staff to provide input into the development of program documentation, training, procedures, and overall structure of the program. The Welcome Baby Community Outreach Specialist will follow Welcome Baby protocols as developed by First 5 LA and will complete other duties as assigned. This is a part-time position, 30 hours per week- Grant Funded.

The Hospital Liaison will have experience conducting standardized family assessments. The Hospital Liaison will introduce the Welcome Baby program and describe the opportunities and services to patients during the hospital visit. The goal of the hospital visit is to invite patients to participate or continue participation in the program; observe parent-infant interaction when possible; assess for social support, infant feeding, and maternal depression; and complete the Bridges for Newborn Screening Tool. The Hospital Liaison will also attend off-site Welcome Baby mandatory meetings and trainings. The Hospital Liaison will be certified in Lactation Education (CLE); or certification must be completed within six months of hire; alternately, staff may be certified lactation consultant (CLC) or international board-certified lactation consultant (IBCLC). The Hospital Liaison will work closely with the Welcome Baby Program Manager, Clinical Supervisor, Grant Coordinator/Hospital Liaison and other program staff.

25% of this position will be dedicated to the Outreach Specialist role and 25% will be dedicated to the Hospital Liaison role

ESSENTIAL DUTIES AND RESPONSIBILITIES

Outreach Specialist Key Position Responsibilities:

· Ability to function as a member of an interdisciplinary team and work closely with all program staff to provide a continuum of services.

· Ability to develop relationships with providers and community organizations and provide input on communication and outreach strategies.

· Experience working with families from diverse age, cultural, and ethnic backgrounds.

· Conduct outreach (group and one-on-one) at provider sites and community-based organizations to recruit potential clients.

· Contact potential clients from outreach to encourage participation in the program and enroll them, if appropriate.

· Conduct client intakes, including collecting key information in a sensitive and nonjudgmental manner.

· Identify appropriate resources for clients and provide referrals as needed during the intake process.

· Ability to adhere to all program guidelines, policies and protocols and actively work toward achieving the goals of the program.

· Empathy, warmth, and the ability to establish ongoing supportive relationships with an ethnically diverse population.

· Participates in appropriate community meetings and marketing and communications efforts as needed.

· Provides input on communication and outreach strategies.

· Develops trust and rapport with clients and provides support using an empathic and client-centered approach.

· Completes all needed paperwork for each client in a timely and organized manner, including clear and well written progress notes.

· Provides intake data to the Clinical Supervisor and administrative support staff in a timely manner for client enrollment and assignment.

· Updates and shares community resources.

· Attends and participates in all staff meetings, bi-weekly group reflective supervision, and individual group reflective supervision.

· Participate in periodic Peer to Peer workshops, webinars and conference calls; and Welcome Baby and Home Visitation regional meetings.

· Complete Welcome Baby training required for Outreach Specialist within one year of hire

Hospital Liaison Kep Positon Responsibilities:

· Responsible for approaching prospective clients at bedside to introduce the Welcome Baby program, verifies willingness to participate, obtains consent, and gathers referral information.

· Responsible for developing a file for each client recruited into the program, documenting all contacts with the family and agencies and completing data forms and program documentation.

· Responsible for developing trust and rapport with clients and provides support using an empathetic and client centered approach.

· Responsible for adhering to all program guidelines, policies and protocols and actively works toward achieving goals of the program.

· Responsible for the conducting an interview with each client recruited into the program, observing parent-infant interaction; assessing involvement of father or other secondary caretaker, level of social support, infant feeding and maternal depression; and provides breastfeeding support

· Assists moms and infants with breastfeeding, education and resources

· Effectively conducts required client assessments using standardized tools for maternal depression and a risk screening interview.

· Works closely with the Program Director, Department Director and nurses to provide a continuum of services.

· Confers with assigned nurse regarding need for any additional hospital services or referrals and assists with referral process, if needed.

· Collaborates and acts as a liaison with other members of the healthcare team, including nurses, lactation specialists, social workers, and physician to coordinate provision of high quality patient care.

· Attends and completes all trainings, including successful completion of CLE training program within 6 months of hire.

· Attends and participates in all home visitation staff meetings, including bi monthly group reflective supervision.

· Completes all needed paperwork for each client in a timely and organized manner, including clear and well written progress notes.

· Independently recognizes and performs duties which need to be done without being directly assigned. Establishes priorities; organizes work and time to meet them.

· Recognizes and responds to priorities, accepts changes and new ideas. Has insight into problems and the ability to develop workable alternatives.

· Accepts constructive criticism in a positive manner.

· Adheres to attendance and punctuality requirements per hospital policy. Provides proper notification for absences and tardiness. Takes corrective action to prevent recurring absences or tardiness.

· Uses time effectively and constructively. Does not abuse supplies, equipment, and service.


POSITION REQUIREMENTS

A. Education

· Bachelor’s degree in Child Development, Social Work, Psychology, Human Development, Public Health, Human Services, Women’s Studies or a related field, plus experience.

B. Qualifications/Experience

· At least one year of experience in community outreach.

· Background in conducting family assessments preferred.

· At least one-year previous work experience in maternal and newborn health services.

· Current BLS (AHA) required.

· CLE Certified or obtain certification within 6 months of hire

C. Special Skills/Knowledge

  • Bilingual preferred.
