Your Role as a Primary Care Physician (PCP)
- The PCP takes full accountability for patient care and outcomes for a panel of up to 450 patients, fostering trust and helping patients alter their health behaviors to improve outcomes.
- Act as a health coach; collaborate with patients in setting health goals and consistently follow up to ensure progress.
- Lead your care team, which includes a medical assistant, care facilitator, and care coordinator, to ensure high-quality patient care.
- Engage with specialists as necessary while remaining actively involved in patient care and recovery.
- Participate in patient admissions for hospitalized patients and engage with the hospitalist to ensure continuity of care.
- Contribute to the management of your center and support the recruitment efforts for patients and potential providers.

Essential Skills
Availability to patients to foster trust and respect, including timely responses to visit requests and communications.Deliver quality care while embracing a service-oriented mindset that prioritizes the well-being of patients.Stay informed on the latest evidence-based medicine and recognize the roles of lifestyle and nutrition in health outcomes.Demonstrate physician leadership to continuously improve team dynamics and healthcare delivery.Engage in self-care practices for personal wellness and improved patient interactions.Proficient with Microsoft Office Suite and other relevant software.Qualifications
