Your Role as a Primary Care Physician (PCP)

• The PCP takes full accountability for patient care and outcomes for a panel of up to 450 patients, fostering trust and helping patients alter their health behaviors to improve outcomes.
• Act as a health coach; collaborate with patients in setting health goals and consistently follow up to ensure progress.
• Lead your care team, which includes a medical assistant, care facilitator, and care coordinator, to ensure high-quality patient care.
• Engage with specialists as necessary while remaining actively involved in patient care and recovery.
• Participate in patient admissions for hospitalized patients and engage with the hospitalist to ensure continuity of care.
• Contribute to the management of your center and support the recruitment efforts for patients and potential providers.
