As a Registered Dietitian or  Dietetic Technician   at Apple Valley Village Health Care Center , you will collaborate with our on-site leaders and culinary team to use your clinical expertise to develop a plan for each resident to help achieve their goals.  Additionally, you will be a part of an established team where your clinical knowledge and critical thinking will be an asset.

Position Type :  Full-Time, Benefits eligible position working days

Wage Range :   $26.79 - $36 / hour depending on experience

Registered Dietitian or  Dietetic Technician Responsibilities :

• Coordinate nutritional care of patients and patients by completing nutritional assessments, MDSs and care plans.
• Conducts a nutrition assessment for patients at nutritional risk per facility policies and procedures
• Develops and implements an individualized plan for nutrition intervention based on nutrition assessment and evidence-based standards
• Collaborate with the interdisciplinary team in meetings and / or rounds
• Experience with electronic charting, the Nutrition Care Process and writing PES statements to provide appropriate documentation for the plan of care
• Works cooperatively with food service staff to ensure conformance to diet prescriptions
• Demonstrates accountability for the proper use of patients protected health information
• At least one year of acute care dietitian experience OR heavy in-patient clinical internship rotations
• Participate in care planning meetings, as required
• Provide nutritional counseling and education for patients and family
• Meet DOH / state / federal and company standards
