As a Community Health Worker (CHW) , you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team-based structure and spend the majority of their time engaging directly with members telephonically. The Community Health Worker also addresses social determinant of health such as transportation, housing, and food access.

Schedule : Schedule is Monday through Friday from 8 : 00 am to 4 : 30pm Central Standard Time. This position is a field-based position with a home-based office.

Location : Brown County, Waukesha County, Ozaukee County, Milwaukee County, WI and surrounding areas.

Local travel up to 50% and mileage is reimbursed at current government rate.

The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
