Job description

Summary : The Certified Nursing Assistant is a member of the health care team who works under the supervision of a licensed nurse or QMA to perform basic nursing care tasks as assigned.

Reports To : Charge nurse of the unit and / or Director of Nursing / Director of Clinical Services

Professional Qualifications :

  • Must be Certified Nursing Assistant or in a State recognized and approved training program to become a Certified Nursing Assistant.
  • Actively on State registry and without any findings.
  • If certified in a State other than Indiana, must be an applicant to take the applicable State Nursing Assistant certification exam.
  • Must have the ability to read, write and follow written instructions and document care given.
  • Must possess a current health certificate / examination required by State agencies.

Required Skills and Abilities :

  • Demonstrate patience, tact in dealing with residents, families, representatives, visitors, and employees.
  • Must display good emotional health and be capable of extended periods of standing, bending and heavy lifting.
  • Capable of following instructions and able to complete records under supervision.
  • Ability to move throughout most of the working day and to perform repetitive lifting and movement of residents.
  • Ability to communicate orally and in writing.
  • Willingness to work with realization that errors may have serious consequences for residents.
  • Willingness to perform a variety of simple repetitive tasks.
  • Ability to work under supervision and to cooperate with other staff.

    • Physical, Sensory, and Communication Requirements :

  • Ability to read and write in English, ability to communicate with residents, families, personnel and support agencies and ability to remain calm under stress.
  • Bending, lifting, walking, grasping, fine hand coordination, ability to distinguish smells, tastes and temperatures, ability to discern colors, ability to hear and respond to pages.
  • This position is a job classification / category for which the employee's duties will cause the employee to have occupational exposure to blood or other potentially infectious material (i.e., body fluids)

    • Essential Job Functions :

  • Observe and report any changes in resident's health status to the Charge Nurse on an ongoing basis.
  • Provide assistance with activities of daily living (i.e., bathing, dressing, oral hygiene, skin care) according to plan of care.
  • Report to supervisor at the beginning and end of each tour of duty.
  • Attend all in-service education programs required of the nursing assistants.
  • Report to supervisor immediately when observing any unusual or significant change(s) in a resident's physical or behavioral condition.
  • Document all care given and observations made while providing care.
  • Contact Charge Nurse to report absences, schedule or change lunch or rest breaks, leave early, change work schedule, change work station and resolve job related problems.
  • Bathe residents as instructed.
  • Provide complete baths at frequency stated in nursing care policies, or individual resident plan of care.
  • Provide partial bath on days when complete bath is not given.
  • Bathe as necessary when bed linen and / or clothing is wet or soiled.
  • Provide extra care to the skin of incontinent and bedfast residents; those who have been sitting in chairs for long periods of time and as instructed.
  • Assist residents to and from chairs and into and out of bed, always using good body mechanics.
  • Assist with ambulating residents, as instructed.
  • Assist with ambulating residents using special devices, as instructed.
  • Change positions of bedfast residents at least every two (2) hours, using good body alignment and anti-contracture devices as instructed.
  • Shave, trim, shampoo and comb hair of residents as instructed.
  • Clean and trim nails, as instructed.
  • Assist residents with oral hygiene, as instructed.
  • Toilet residents, as needed.
  • Clean and store equipment and personal care items. When caring for a resident, the CNA is responsible for keeping the room clean and in good order (including making beds).
  • Observe all infection control procedures as instructed.
  • Ensure fresh ice is available to resident, unless otherwise instructed.
  • Assist with meal service (staff shall encourage each resident to be as independent as possible).
  • Assist residents to designated eating areas.
  • Serve food trays; feed and assist those needing assistance as instructed.
  • Report to your supervisor any change in resident's appetite. Monitor all diabetics carefully, notify nurse of decreased meal consumption.
  • Accurately record meal intake as designated.
  • Provide residents with post-meal grooming assistance as needed and clean dining areas after meal service, as instructed.
  • Treat all residents with dignity, kindness and respect.
  • Every call light and alarm is yours to answer promptly and courteously. Consider each light an emergency until answered.
  • Pass nourishments and snacks to residents as assigned and report to nurse those who refuse.
  • Obtain weights and heights, as scheduled.
  • Properly apply, release and document restraints, as instructed.
  • Provide range of motion to residents as instructed during a.m. and p.m. care.
  • Ensure privacy is afforded to the resident.
  • Accurately record intake and output as instructed.
  • Position all residents' tubings and collection bags, as instructed.
  • Wash hands before and after providing direct resident care.
  • Provide proof of annual Nurse Aide Registry renewal.
  • Function within the limits of Nurse Aide Training.
  • Comply with all Federal and State regulatory requirements.
  • Implement the resident's care plan as assigned by the supervisor.
  • Receive and complete resident care assignments and other duties as assigned.
  • Seek assistance of supervisor when unsure of assignment, procedure, or situation.
  • Report immediately malfunctioning call lights and other equipment.
  • Assist residents in care for their clothing and personal property.
  • Clean up all spills immediately, placing caution signs, if indicated.
  • Keep information concerning your residents confidential.
  • Perform other duties as directed by the supervisor or other professional nursing staff that fall within the scope of practice of the nursing assistant.
