Role Description

This is a full-time, on-site role for a Home Health Licensed Practical Nurse (LPN) located in Lancaster, PA. The Home Health LPN will be responsible for providing direct patient care in the home setting, including administering medications, monitoring patient health, and collaborating with other healthcare professionals. Additional duties include educating patients and their families on disease management and post-treatment care, as well as maintaining accurate patient records.

Qualifications

• Licensed Practical Nurse (LPN) certification and Nursing skills
• Knowledge of Medicine and Home Care practices
• Experience in providing direct Patient Care
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Previous experience in home health care is a plus
• Current CPR certification
• Reliable transportation and valid driver's license
