The West Portland / Hillsboro, Oregon Veterans Administration Community Based Outpatient Clinic is seeking a full-time B oard-Certified / Board Eligible Family Practice or Internal Medicine physician for its out-patient clinic in Hillsboro, Oregon for our Primary Care Staff opportunities .  Being part of Portland VA Medical Center that provides services to an integrated network of care facilities, and a distributed system of outpatient clinics and hospitals located throughout Oregon.  The PVAMC is a 303-bed medical center providing primary care to local veterans, as well as a quaternary referral center for veterans in Oregon, southern Washington, and parts of Idaho. Last year 80,000 patients were treated in the PVAMC system.

The Health Care Clinic operates under a PACT Model (Patient Aligned Care Team). Together with his or her team, comprised of a PCP, RN, LPN, and a Medical Support Assistant, the PCP cares for a panel of approximately 1200 patients. The PACT provides comprehensive care for our Veteran population, including the diagnoses, care, and treatment of both acute and chronic conditions. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. The position is Monday thru Friday, no weekends, and no holidays.

Candidates must be board certified in Family Practice or Internal Medicine and have demonstrated leadership and have administrative capabilities.  Must be strongly motivated in promoting appropriate staff education and development programs, and demonstrate a commitment to professional enrichment, merit promotion, equal employment opportunity and positive labor relations.
