Adventist HealthCare seeks to hire an experienced Manager of Care Navigation for our Case Management department who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing.

As Manager of Care Navigation, you will :

• Perform duties to conduct and manage the day-to-day operations of the case management / social work functions, communicating with departmental staff to facilitate daily department functions
• Review and analyze activities, costs, operations, and forecast data to determine progress toward stated goals and statistical / financial purposes
• Promote compliance with federal and state regulations and contractual agreements
• Develop, implement, and maintain compliance with policies and procedures regarding case management
• Oversee compliance with the final rule and the clinical team
• Develop, implement, and maintain case management programs to facilitate the use of appropriate medical resources and decrease health plan financial exposure
• Facilitate ongoing communication between case management staff, members, contracted providers, and subsidiaries
• Develop staff skills and competencies through training and experience

Work Schedule : Full-Time, Day Shift

Pay Range :

$83,112.12 - $134,035.20

In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.

Tobacco and Drug Statement

Tobacco use is a well-recognized preventable cause of death in the United States and an important public health issue.
