Key Responsibilities :
- The Family Medicine Physician (FMP) will provide comprehensive care to patients with acute and chronic conditions, specifically catering to older adults.
- The FMP will assume full responsibility for patient care and outcomes, collaborating with specialists as needed while maintaining involvement in their patients' detailed care.
- Engagement with hospitalists during patient hospitalization is expected, regardless of affiliation with ChenMed.
- Responsibilities include assessment, diagnosis, treatment, health education, promotion, care coordination, and thorough documentation for patients with complex health needs.
- The FMP will lead a care team that includes care promoters, care facilitators, and care coordinators for patients visiting the office.
- For homebound or facility-based patients, the FMP will work with the transitional care team and various resources available in the market.
- Active participation in the management of their center is required, including covering for colleagues as needed.
- Supervisory collaboration with Advanced Practice Practitioners (APP) will be part of this role.
- Other duties as assigned by management.
Qualifications for Success :
Availability for patients is crucial to build trust; PCPs should be accessible for walk-ins and timely communication.A strong service orientation is essential; treat every patient interaction with the same care you would wish for family.Knowledge of evidence-based medicine is important, with recognition of the impact of lifestyle and nutrition on health outcomes.Leadership skills must be continuously developed for the benefit of patients, teams, and the organization.Dedication to quality care and ongoing improvement is paramount for achieving high-quality outcomes.Effective influence over patients, teams, and oneself will lead to optimal results.Self-care among physicians is vital for sustaining the health of everyone involved.Proficiency in Microsoft Office Suite and ability to operate in a Windows environment are required.Must be willing to travel locally as necessary, including for training and recruitment.Fluency in English is required; additional language skills may be necessary in some centers.This position requires independent judgment and excellent communication skills.Requirements :
Doctorate in Medicine (MD) or Doctor of Osteopathic Medicine (DO) specializing in Internal Medicine, Family Medicine, Geriatrics, or related field.Active State Medical License is required; eligibility for licensure must be established.Preferred Board Certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.Current DEA number is necessary for prescription writing.Basic Life Support (BLS) certification is required within the first 90 days.Compensation :
Annual salary ranges from $214,700 to $306,714.
