Position Summary :

V-Tech Solutions, Inc. is seeking compassionate and organized Case Managers to support client care coordination and service delivery in Washington, DC. The Case Manager will play a critical role in assessing client needs, developing treatment plans, coordinating services, and advocating for resources to support client well-being and recovery.

Requirements :

  • Education : Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred)
  • Certification : Licensed or certified in case management or social services (as applicable)
  • Skills & Abilities :
  • Strong interpersonal and communication skills
  • Ability to conduct intake interviews and assess therapeutic needs
  • Experience coordinating multidisciplinary services and resources
  • Proficiency in documentation and record-keeping
  • Ability to manage crises and advocate for client entitlements
  • Familiarity with treatment planning and discharge coordination
  • Basic computer skills for data entry, reporting, and communication

Responsibilities :

  • Admit new clients by reviewing records and conducting orientations
  • Complete intake interviews and determine therapeutic, medical, and psychosocial needs
  • Develop individualized treatment programs and coordinate service schedules
  • Monitor client progress through attendance tracking and treatment evaluations
  • Advocate for services, intervene in crises, and provide personal support
  • Maintain accurate case notes and progress logs
  • Facilitate weekly interdisciplinary meetings and communicate updates to teams and families
  • Coordinate discharge planning and post-discharge support
  • Educate and train family members on care plans and available resources
  • Support staff development through training and educational resources
  • Evaluate treatment outcomes and recommend process improvements
  • Monitor financial trends and contribute to budget planning
  • Prepare reports and grant applications based on treatment data and outcomes
  • Stay current through professional development and networking
  • Contribute to organizational growth by embracing new initiatives and responsibilities

    Compensation : Pay Range :
