Description :
As a continuing care LPN, providing top-notch patient care is paramount.
Under the direction of an RN, mid-level Provider, or Physician, you'll be responsible for assessing, planning, implementing, and evaluating patient care.
You will be a vital part of the team, making a real difference in the lives of patients and their families.
Essential Functions :
Collect and monitor assessment data and report physiological, psychological, sociocultural, and spiritual needs of patients and families.
Provide direct patient care under the direction of the RN, mid-level Provider, or Physician, consistent with policies and procedures.
Inform patients and families about treatments and medications being received and equipment being utilized.
Document care provided via computer and written patient records in a manner consistent with guidelines.
Greet and communicate with patients and families in a manner consistent with caring and respect.
Collaborate with other members of the healthcare team to ensure continuity of care.
Clean the patient's environment, work area, and equipment, and report any equipment malfunctions.
Document patient chargeable items according to standards.
Participate in selected quality monitoring activities under the direction of the RN, mid-level Provider, or Physician.
Demonstrate a high standard of moral and ethical behavior.
Advocate for privacy, confidentiality, and security of patient and staff data.
Actively participate in safety initiatives and complete all position-related competencies in a timely manner.
Perform other duties as assigned.
