Compensation : $70K-75K pr / yr
Job Type : Full-Time and In-person position, 7am-3pm, Monday - Friday
Collaborating with aLong-Term Care / Rehabilitation Facility in the Bronx, NY that isseeking a Social Worker
Social Worker Duties and Responsibilities :
- Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
- Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources
- Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
- Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
- Liaison between family and other interdisciplinary team members
