Job Description
A Long Island nonprofit services agency is looking to add a new, licensed Registered Nurse (RN) or licensed Practical Nurse (LPN) to join their growing team on a full-time basis at one of their residential facilities in Suffolk County.
About the Opportunity :
- Setting : Residential
- Shift : Day
- Schedule : M-F 7am to 3pm OR M / W / F with Friday being 7am to 7pm
- Patient Population : 6 to 21 year old individuals with ID / DD
Responsibilities : The Nurse will :
Develop a person-centered plan of care that prescribes strategies and interventionsSupport discharge planning process with members of relevant teamsProvide or coordinate the resident / family educationCollect comprehensive data pertinent to the resident's health or the situationPerform all other duties as assignedQualifications :
6+ months of related Nursing experienceGraduate of an accredited School of NursingNYS Nurse (RN or LPN) licenseBLS certificationStrong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical ComplexitySolid assessment, clinical and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills :
Bachelor's Degree in NursingNational Certification in area of specialty
