Position Overview
As a Physical Therapist (PT), you will evaluate patients and develop individual plans of care to improve their mobility, strength, and overall functional independence. You will work closely with other therapy team members and medical staff to support each resident’s unique goals.
Key Responsibilities
Conduct evaluations and design treatment plans tailored to individual needs
Implement and document skilled physical therapy interventions
Monitor patient progress and revise plans as needed
Collaborate with interdisciplinary team members to optimize care
Educate patients, families, and caregivers on exercises and strategies
Maintain timely and accurate documentation per company and regulatory guidelines
Participate in care planning meetings and department initiatives
Ensure a safe and clean therapeutic environment
Qualifications
Active Texas Physical Therapist license
In good standing with all relevant licensing boards and agencies
Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
Familiarity with Net Health documentation system a plus
Excellent interpersonal, communication, and organizational skills
