Colorado Licensed LPN Nurse

A company is looking for a Spanish Speaking LPN Nurse (REMOTE) in Colorado.

Key Responsibilities

Conduct comprehensive assessments of patients' health needs

Collaborate with healthcare teams to develop and update care plans

Educate patients on chronic disease management and coordinate care across providers

Qualifications & Requirements

Active Colorado Licensed Practical Nurse (LPN) license in good standing

Fluency in Spanish and English (oral and written)

At least 2 years of experience in patient care or chronic disease management

Experience with Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) programs preferred

Proficiency in EHR systems and familiarity with RPM tools
