Field-based, making Primary Care House Calls, Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes, Collaborate with a multidisciplinary team to deliver integrated care. Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients, Fluent Spanish is ideal.

Job Responsibilities :

Field-based, making Primary Care House Calls.

Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes.

Collaborate with a multidisciplinary team to deliver integrated care.

Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients.

Fluent Spanish is ideal.

Qualification / Experience :

Valid Georgia ARNP license.

Reliable transportation and current state driver's license and proof of auto insurance.

Current DEA license.
