Care Navigation Coordinator

A company is looking for a Care Navigation Resource Coordinator.

Key Responsibilities

Support the care navigation team by communicating with post-acute providers and assisting in discharge planning

Schedule and reschedule follow-up appointments for high-risk patients as needed

Collaborate with vendors, physician offices, and clinics to ensure smooth transitions for patients

Required Qualifications

High School Diploma

2-3 years of clerical experience in a medical setting

Ability to adapt to change and support process improvements

Working knowledge of medical terminology

Computer literate with understanding of Microsoft Office products and electronic medical records
