Pennsylvania Care Management Coordinator

A company is looking for a Care Management Support Coordinator II to join their team.

Key Responsibilities

Conduct telephonic outreach to members for enrollment into Case Management programs

Provide support to members in connecting with health plan and community resources

Document and maintain non-clinical member records in compliance with state and regulatory requirements

Required Qualifications

High School diploma or GED required

1 - 2 years of related experience in a healthcare setting

Knowledge of medical terminology

Proficiency in Microsoft Office applications

Must reside in Pennsylvania
