Performs diagnostic assessments and collect s diagnostic information to determin e an individualized treatment plan for the client. Plan s treatment modalities, coordinate s treatment plans and follow s clients through the treatment process . Provides counseling, problem-solving and community support (individual and group). Requests consultation for clients that includes but is not limited to providers, social workers , and nursing staff when necessary. Evaluate the client's treatment plan through follow-up assessment and makes adjustments based on clinical findings. Requires bachelor's degree with at least 30 semester hours or equivalent quarter hours in a human services field, including but not limited to sociology, social work, counseling, psychology, or human services. Valid driver's license. Basic Life Support (BLS) certification within 6 months of hire . Crisis Prevention Institute (CPI) training within 6 months of hire. Experience working with persons with severe and persistent mental illness. Specific offers are determined by various factors , such as experience, skills, internal equity, and other business needs. T he salary range listed does not include other forms of compensation which may include bonuses / incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.

Shift : Monday-Friday; 8 : 00 AM - 4 : 30 PM
Location : This position will cover multiple counties in Iowa including, Scott, Muscatine, Clinton, and Cedar
The Employment Specialist provides case management and employment-based services to severely mentally ill adults in the community. The Employment Specialist manages care under the direct supervision of a Licensed Mental Health Professional (LMHP) or Qualified Mental Health Professional (QMHP) and performs diagnostic assessment, connects clients to competitive employment, coordinates treatment planning, and monitors clients through the treatment process. The Employment Specialist facilitates coordination of care with mental health team(s) to optimize individual treatment plans and safety of the behavioral health care client.
