ABOUT THE POSITION
This full-time role (40 hours / week) provides structured, one-on-one, strengths-based support to youth in coordination with the HUB behavioral health provider to implement treatment and care plans addressing mental health, behavioral, and emotional needs. The Therapeutic Mentor engages youth through social, recreational, educational, and daily living skill-building; maintains consistent contact with caregivers and collaterals; documents progress toward goals; participates in care planning meetings; advocates for resources; and completes required paperwork and data entry on time.
KEY RESPONSIBILITIES
- Deliver one-to-one, skills-focused mentoring aligned to the youth’s treatment and care plans developed by the HUB provider.
- Engage youth in therapeutic interventions through social, recreational, educational, and daily living skills to promote behavioral and emotional growth.
- Maintain regular contact with caregivers, HUB clinicians, schools, and other collaterals; document progress toward identified goals.
- Participate in Care Planning Team meetings; communicate observations and recommend adjustments to goals / strategies.
- Identify, advocate for, and help access community resources that support youth mental and emotional health needs.
- Complete timely, accurate documentation and data entry per payer and agency standards; uphold confidentiality and HIPAA.
- Provide services that are culturally responsive, trauma-informed, and family-centered; model professional boundaries and safety.
REQUIREMENTS
Must-Haves
Associate’s or Bachelor’s degree in a human services field preferred; equivalent experience considered.At least 1 year of experience working with children, adolescents, or transition-age youth.Age 21+ with a valid, unrestricted driver’s license and reliable transportation for community-based work.Strong communication, engagement, and documentation skills; ability to follow written and verbal directions effectively.Commitment to equity, cultural responsiveness, and strengths-based practice.Nice-to-Haves
