Description\n\nPrimarily responsible to plan, organize, direct home care services. Experience in nursing, with emphasis on community health education / experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.\n\nPosition : Start of Care / Admission Registered Nurse (RN) for Home Health\n\nWage range : $88,000-$90,000 annually, depending on experience\n\nService Area : Twin Cities Metro Area. Plus, mileage reimbursement of .70 cents per mile.\n\nSchedule : Full-Time / Monday - Friday, 8 : 00am-5 : 00pm (with 2 holidays per year rotation and NO WEEKENDS!)\n\nLifespark is a complete senior health company headquartered in St. Louis Park, Minnesota. • Frame the discussion and builds understanding with clients that Lifespark starts with the clients' immediate needs and wants (short term) while also working to build a longer-term plan to help the clients avoid the roller coaster of health care crisis thereby protecting and improving their independence and quality of life.\n
• Effectively articulate and differentiate the Lifespark Life Care Management model.\n
• Provide assurance and relief (peace of mind) to clients and families that Lifespark will effectively address their needs and wants.\n
• Make the clients feel comfortable and well-cared for.\n
• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).\n
• Initiates the plan of care\n
• Uses health assessment data to determine nursing diagnosis.\n
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.\n
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.\n
• Counsels the patient and family in meeting nursing and related needs.\n
• Provides health care instructions to the patient as appropriate per assessment and plan of care.\n
• Identifies discharge planning needs as part of the care plan development.\n
• Completes necessary paperwork accurate at admission and submits to medical records the same day.\n
• Completes emergency preparedness plan with client / caregiver at admission visit\n
• Communicates with IDT post admission or non-admission including client status and a summary of the plan of care for client.\n
• Ensure positive / professional communication on team\n
• Introduction / Establish Rapport\n
• Customer service resolution; escalate concerns\n
• Prepares clinical notes and updates the primary physician\n
• Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains / receives physician's orders\n
• Communicates with community health related persons to coordinate the care plan\n
• Honors Client Confidentiality, Rights, Privacy, and Reporting Maltreatment Expectations\n
• Understands and Accepts Clients' Diagnosis\n
• Understands and accepts emotional needs of client\n
• Accepts unique client symptoms and behaviors\n
• Follows all emergency preparedness and response procedures\n
• Identifies home safety issues and cascades info to the IDT\n
• Notifies IDT of emergency preparedness plan\n
• Demonstrates proper procedures for handling, lifting, and transfers\n
• Overview of Process, Types of Reports and Forms\n
• Enters complaints, occurrences into EMR as necessary at skilled visits\n
• Ensures education on how to use equipment safely is completed at admission and that medical supplies are ordered.\n
• Plots necessary supervisory visits per regulations\n
• Thoroughly reconciles all medications\n
• Completes all documentation by 10am the day following the admission or skilled visit\n
• Complete orientation and annual training requirements.\n
• Promote safety in the workplace through real time coaching, participation in safety awareness training\n
• Participate in After Hours on-call program\n
• Other duties as assigned\n\nQualifications : \n
• 2 years' RN experience in health care (preferably home care or hospice)\n
• Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred\n
• Experience in customer service / direct member contact field preferred\n
• Experience supervising caregivers\n
• Knowledge of Medicare Home Health & Hospice regulations\n
• Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.\n
• Graduate of an accredited school of nursing\n
• Current Minnesota Nursing License\n
