A company is looking for a Field RN Case Manager.

Key Responsibilities

Manage care for patients in a remote setting, ensuring effective communication and coordination

Conduct discharge planning and home health care coordination

Collaborate with healthcare teams using virtual tools and systems

Required Qualifications

Current unrestricted IL state licensure as an RN, LCSW, or LCPC

Minimum 3-5 years of clinical practical experience

Minimum 2-3 years of experience in care management or home health care coordination

Unrestricted driver's license and access to a car

Proficiency in standard corporate software applications
