Position : Primary Care Physician, IntuneHealth
Role Overview : As a Primary Care Physician (PCP) with IntuneHealth , you'll manage a panel of up to 550 patients, delivering exceptional care through our innovative model. You ll act as a health coach, not just a consultant, guiding patients to set and achieve their health goals. Your role will involve :
- Patient Care : Provide comprehensive care for patients with acute and chronic conditions. Take full accountability for patient outcomes, engaging specialists as needed.
- Care Coordination : Work with care teams to ensure high-quality, coordinated care across various settings including in-center, in-home, and telehealth.
- Leadership and Culture : Lead your care team to achieve targets, foster a positive culture of accountability and passion, and actively participate in center management and recruitment efforts.
- Continuous Improvement : Stay updated on best practices and contribute to improving clinical quality and patient outcomes.
