Day to day as a Licensed Social Worker

As a new Licensed Social Worker at Minden Medical Center, you can expect to conduct intake assessments, develop individualized care plans, provide counseling and support to patients, collaborate with healthcare teams, and participate in case conferences. Your day-to-day responsibilities will involve documenting patient interactions, coordinate discharge plans, and connecting patients with community resources. This full-time position operates on a Monday through Friday schedule, providing you with a structured work week to make a meaningful impact in the lives of those you serve.

What we're looking for in a Licensed Social Worker

To excel in the role of Licensed Social Worker at Minden Medical Center Minden Physician Practices, you must possess strong organizational and communication skills. Proficiency in patient assessment and resource coordination is essential. The ability to participate effectively in department meetings, while maintaining timely and accurate charting, is crucial for success in this position. Building and nurturing positive relationships with healthcare professionals and team members is key to ensuring patient care is delivered in a safe, efficient, and compassionate manner.

Knowledge and skills required for the position are :

  • Identify needs of patients.
  • Aide in helping with financial / insurance resources.
  • Must complete charting in a timely manner.
  • Maintains a good working relationship with physicians, nurse managers, department directors and other healthcare team members to ensure the delivery of quality care in a safe and effective manner.
  • MUST BE LPC or LCSW in the State of Louisiana.
