Join a team that values compassion, quality, and the difference you make every single day . ‍⚕️ Your Impactful Role as an LPN : As a Licensed Practical Nurse with Loyal Home Care Services, you'll be at the center of personalized care plans - delivering top-tier support and forming trusted relationships with clients and families. Responsibilities :

• Deliver skilled nursing care based on the Plan of Care and within scope of MA Nurse Practice Act
• Support and carry out care plans initiated by the Registered Nurse
• Provide education and guidance to patients and families on condition management and at-home care
• Monitor health status, document accurately, and report updates to the clinical team
• Collaborate with RNs and coordinate additional services (PT, OT, HHA, MSW, etc.)
• Use medical equipment safely, maintaining infection control standards
• Stay current on best practices through ongoing clinical development
• Be an advocate and source of encouragement for your patients What You'll Need to Join Us :
• Active LPN license in Massachusetts
• Graduate of an accredited LPN or vocational nursing program
• Minimum of 3 years nursing experience (home health or med / surg preferred)
• Current CPR / BLS certification
• Reliable transportation and valid driver's license
• Up-to-date health certificate & TB test
• Fluent in English (spoken and written)
• Demonstrated knowledge of HIPAA & patient confidentiality
• Must be 18 years or older and authorized to work in the U.S. Perks & Benefits
