POSITION SUMMARY / RESPONSIBILITIES

Serves as a member of the outreach team implementing the Healthy Families Bexar grant program. Healthy Families is an evidence-based home visiting program that promotes child well-being through family-focused support provided in the home. This position will conduct home visits to provide support and connect families to services and resources available. This position will recruit families, help develop community partnerships and implement the Healthy Families curriculum provided. Additional activities will include patient tracking, data entry, and reporting.

EDUCATION / EXPERIENCE

Bachelor’s degree in early childhood development, social sciences or health education is preferred or related field of study. A minimum of 1 years or equivalent experience in the area of social services, health education, public health, or case management. Experience in public speaking and educational presentations demonstrating effective communication skills is required. English / Spanish fluency is strongly desired.
