  • Bluffton / Warren, IN
  • Fort Wayne, IN - multiple locations throughout the city

Georgetown

  • Southwest- Illinois Rd.
  • New Haven
  • Rudisill
  • LaGrange, IN
  • Ligonier, IN
  • Noble / Kendallville, IN
  • Goshen, IN
  • Wabash, IN
  • Warsaw, IN

    • Details :

  • Hours of practice are usually Monday-Friday, 8 a.m. 5 p.m.; some extended hours may be required (varies by location)
  • Light physician call the call center allows for manageable call with excellent work / life balance!
  • Outpatient only
  • Utilize Epic EMR
