Job Overview
The MTM Pharmacist delivers telephonic Medication Therapy Management (MTM) services that directly improve member outcomes by identifying and resolving medication-related problems. This pharmacist-led service focuses on performing medication reconciliations with high-risk members, assessing therapy effectiveness, and communicating findings to members, caregivers, and prescribers. The MTM Pharmacist collaborates closely with physicians, nurse case managers, and internal teams to optimize medication regimens based on evidence-based guidelines, supporting members on their path to better health.
Key Job Duties
- Conduct comprehensive telephonic medication reviews to ensure complete medication reconciliation and assess member therapy for clinical appropriateness.
- Identify, document, and coordinate the resolution of medication-related problems.
- Prepare clear, patient-friendly written summaries (takeaways) for each member interaction.
- Draft and send prescriber-facing communication (faxed materials) to support problem resolution and care coordination.
- Build and maintain strong relationships with internal and external stakeholders, including members, prescribers, and nursing staff.
- Participate in continuous quality improvement initiatives and other assigned responsibilities as directed by leadership.
