Description :

As a member of the interdisciplinary team, you will contribute professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.

Promote the use of evidence-based practice through the nursing process, protocols, and procedures.

Assess patient's status through interview, observation, and physical assessment of exact problems and outcomes.

Develop an individualized plan of care using a collaborative approach with the patient / family and healthcare team.

Use appropriate interventions identified in the plan of care to achieve expected patient outcomes.

Systematically evaluate and document patient's progress toward expected outcomes / goals on an ongoing basis throughout the shift.

Modify the plan of care based on evaluation andas needed based on changes in patient condition.

Complete required documentation, including patient chart and event reports.

Demonstrate professional accountability for your practice by seeking resources and continued self-development.

Demonstrate technical knowledge of equipment and troubleshoot problems when they arise.

Acquire and maintain current knowledge and skills in evidence-based nursing practice.

Promote the professional development of yourself and team members.

Share clinical knowledge through informal teaching and in response to the learning needs of others.

Demonstrate a practical problem-solving approach to patient moving and handling issues when applicable.

Demonstrate skills to promote patient and staff safety through appropriate use of lift equipment when applicable.

Possess knowledge and skills necessary to obtain adequate lab specimens for testing.

Perform patient bedside lab testing and associated quality control in compliance with medical provider’s orders and hospital procedures / policies.

Coordinate the interdisciplinary care of the patient through collaboration with team members and utilize additional resources when necessary.

Analyze clinical situations systematically and act to obtain resources or correct problems to meet or exceed expected outcomes or patient needs.

Recognize the need for assistance and utilize resources, including consultations, in a timely manner.

Demonstrate ownership and persistence to ensure problems are resolved effectively.

Assume assignment of non-complex and some complex patient management with some turnover in assignment.

Prioritize your work based on changes in patient condition, changes in assignment, and unanticipated interruptions.

Identify and utilize team resources to solve identified problems or needs.

Collaborate with the healthcare team in the organization of tasks and activities for the patient's care from admission to discharge according to the plan of care and to meet identified expected outcomes.

Work with team members in delegating tasks and activities appropriately.

Provide goal-focused shift report.

Advocate with physicians on behalf of patients when necessary.

Utilize communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes and unit productivity.

Choose an appropriate and meaningful communication style when interacting with others and give and accept constructive feedback.

Assess and choose teaching strategies appropriate to the patient's / family's age-specific needs.

Adapt teaching strategies based on the patient's response, readiness to learn, and level of comprehension.

Consider cultural, age-specific, gender, and developmental factors when teaching patients about procedures, illnesses, and hospital experiences.

Ensure patient safety in the hospital environment by following policy / procedures and adhering to National Patient Safety Goals.

Consult appropriate resources within the hospital to provide specialized education.

Utilize patient education systems as a resource for patients and family members.

Actively participate in data collection, analysis, and problem-solving and provide input on process improvements and patient satisfaction.

Ensure that care provided is consistent with standards of care, evidence-based practices, and national patient safety goals.

Work with the interdisciplinary team to identify opportunities for improvement in patient care and patient / family satisfaction.

Actively promote and achieve customer satisfaction.

Promptly and efficiently follow up on patient / family requests / needs and anticipate their needs.

Encourage coworkers in achieving excellent customer satisfaction.

Serve on unit-based councils.

Identify practice problems within the unit.

Implement changes in your clinical practice based on literature and unit process improvement outcomes.

Work on assigned projects, data collection, and audits as assigned.
