The Care Manager RN - Cardiology manages chronic illness patient populations and coordinates care with primary care providers within an ambulatory setting. This role requires a Registered Nurse license, experience in medication administration, and complex patient management, preferably with a Bachelor’s degree in Nursing. Employed by Providence, the RN supports patient-focused, whole-person care in a multi-state healthcare network.

Description

The Care Manager RN is responsible for managing patient populations with chronic illness and / or multiple co-morbidities. This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Required Qualifications :

• Coursework / Training : Graduation from an approved, accredited school of Nursing.
• Washington Registered Nurse License upon hire.
• 2 years of work experience in an RN role.
• Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders.
• Experience managing complex patients in an ambulatory setting.
