Job Description\n

American Traveler is seeking an experienced RN for a hospice position providing in-home patient care across the Tri-County Area, requiring an active SC or compact RN license and a valid driver’s license.

\n \n Responsibilities\n

  • Home health and hospice setting covering the Tri-County Area with specific zip code assignments
  • Patient population includes individuals with terminal illnesses and compromised health states
  • Shifts are Friday, Saturday, Sunday plus one weekday with options for 4x10 hour shifts or a combination of 8 and 10 hour days, totaling a 40 hour work week
  • Schedule is dependent on patient appointment needs, with no routine call requirements
  • Must use own transportation to make home visits
  • Floating may be required to a nearby sister hospital
  • Uses Cerner EMR for documentation
  • Responsible for planning, organizing, and directing comprehensive hospice nursing care in homes or hospice inpatient settings
  • Supervise and coordinate care with family, caregivers, and interdisciplinary team to meet medical and emotional patient needs
  • Administer medications, assess and manage symptoms, and deliver end-of-life care
  • Educate patients and families in self-care techniques and ensure necessary equipment or services are arranged
  • Attend and participate in interdisciplinary team meetings, quality improvement activities, and continuing education
  • Supervise LPNs and Hospice Aides and delegate responsibilities as needed
  • Will require occasional exposure to blood, body fluids, communicable diseases, and moderate workplace noise
  • Assignment limited to one calendar year; a 30-60 day break is required before returning for another contract

  • Active RN license in South Carolina or a compact / multi-state RN license
  • Current AHA CPR certification
  • Graduate of an accredited nursing program
  • Minimum two years of professional nursing experience in public health, acute care, home care, or hospice inpatient settings
  • Valid South Carolina driver’s license with a good driving record
  • Ability to perform physical duties including frequent standing, walking, lifting and transporting patients up to 50 lbs
  • No local candidates within a 50-mile radius of the facility
