Primary Facilities : Little Falls Hospital (LFH) and Cobleskill Regional Hospital (CRH)  Coverage : 3 days per week at LFH, 2 days per week at CRH (subject to caseload needs). Traveling between facilities mid-day is not expected except in rare cases. Basic Details  Start Date : ASAP  Schedule : 40 hours per week, Monday–Friday, 8 : 00 AM–4 : 30 PM  On-Call : None Setting & Caseload  Primary Setting : Critical Access Hospitals o􀆯ering outpatient, acute, and subacute swing bed rehabilitation services.  Caseload : Minimum of 7–8 patients per day, including both outpatients and inpatients, with most being outpatient.  Age Groups : Must be fully competent to treat patients from at least age 3 through advanced age.  Typical Diagnoses : Swallowing disorders (acute and chronic), speech and language impairments, neurological conditions (e.g., strokes, Parkinsonism), developmental delays, dysphagia, fluency, and diet selection / recommendations. The clinician must be fully capable of independently performing both evaluations and treatments in these areas. Requirements & Preferences  Certifications : Candidate must hold CCCs (Certificate of Clinical Competence).  Skills & Experience : Must have experience working independently. Should be skilled in swallow evaluations and treatment, as well as outpatient swallow and language evaluation and treatment. FEES, trach / vent, and MBSS experience are not required as these services are not currently o􀆯ered at LFH or CRH.
