Responsibilities :

Provide skilled nursing care and assessments under the plan of care

Administer medications and treatments as prescribed

Monitor and document patient condition and report changes to physician

Educate clients and family members on disease management, medication, and care needs

Collaborate with caregivers, care managers, and healthcare professionals

Ensure proper infection control practices are followed

Maintain accurate and timely clinical documentation

Qualifications :

Current MA LPN or RN license in good standing

Minimum of 1 year clinical nursing experience (home care experience preferred)

Current CPR certification

Reliable transportation and valid driver’s license

Excellent communication and interpersonal skills

Must have knowledge about Medicare and Medicaid requirements and documentation

Supervisory experience is preferred , especially in a home care or community setting

Ability to work independently and manage time effectively
