About the Role : We are seeking a highly experienced and motivated Licensed Practical Nurse (LPN) to join our leadership team as a Clinical Coordinator . This role is ideal for an LPN with a strong clinical foundation and a desire to grow into the operational and business side of homecare—particularly within the framework of DBHDD services.
Key Responsibilities
- Assist with clinical coordination and office management under RN supervision
- Support case management and contribute to the development and implementation of individualized care plans
- Complete and update HRST assessments, protocols, and documentation
- Provide clinical support and guidance to field nurses and direct care staff
- Participate in leadership functions, including staff training, quality assurance, and compliance monitoring
- Collaborate with interdisciplinary teams to ensure continuity and quality of care
Qualifications
Active LPN license in the state of [Insert State]Minimum 4 years of LPN experience, including roles such as Unit Manager, ADON, or similar leadership positionsBackground in homecare, preferably with experience in IDD / DD populationsFamiliarity with DBHDD regulations, HRST tools, and person-centered planning.
