Job description

Job Opening : Clinical Coordinator (LPN) Location : Atlanta , GA Schedule : Monday–Friday, 9 : 00 AM – 5 : 00 PM Employment Type : Full-Time Reports To : Registered Nurse (RN)

About the Role : We are seeking a highly experienced and motivated Licensed Practical Nurse (LPN) to join our leadership team as a Clinical Coordinator . This role is ideal for an LPN with a strong clinical foundation and a desire to grow into the operational and business side of homecare—particularly within the framework of DBHDD services.

Key Responsibilities

  • Assist with clinical coordination and office management under RN supervision
  • Support case management and contribute to the development and implementation of individualized care plans
  • Complete and update HRST assessments, protocols, and documentation
  • Provide clinical support and guidance to field nurses and direct care staff
  • Participate in leadership functions, including staff training, quality assurance, and compliance monitoring
  • Collaborate with interdisciplinary teams to ensure continuity and quality of care

Qualifications

  • Active LPN license in the state of [Insert State]
  • Minimum 4 years of LPN experience, including roles such as Unit Manager, ADON, or similar leadership positions
  • Background in homecare, preferably with experience in IDD / DD populations
  • Familiarity with DBHDD regulations, HRST tools, and person-centered planning. Willing to train the right candidate
  • Strong documentation, communication, and organizational skills
  • Interest in learning the business and operational aspects of homecare services
