As a crucial member of our team, you will be responsible for ensuring the quality and safe delivery of home health services. You will assess, plan, implement, evaluate, and coordinate all care in the home, working collaboratively with patients and the healthcare team.

Key Qualifications :

• Valid and current license to practice as a Licensed Practical Nurse in New York State.
• At least 1 year of experience in home healthcare or related medical / surgical settings, such as critical care or wound care.
• Strong assessment skills and the ability to respond effectively to the needs of patients and families in the home environment.
• Professional liability insurance as specified by the organization.
• Proficiency in utilizing industry-standard technology, including tablets, when serving patients in the field.
• Experience with Oasis assessments is preferred.
• Must possess a reliable automobile in good condition, available for field use.

Competitive pay rates ranging from $34.46 to $41.48 per hour, based on your experience.
