We seek dedicated Primary Care Physicians who :

  • Take Charge of Patient Outcomes : Assume responsibility for delivering exceptional clinical care and improve health outcomes for a panel of up to 450 patients.
  • Act as Health Coaches : Guide patients in setting achievable health goals and support them throughout their journey toward better health.
  • Promote Actionable Behaviors : Simplify healthcare processes to ensure meaningful behavior changes in patients.
  • Foster a Positive Care Culture : Champion an uplifting environment filled with accountability, love, and passion among care teams.

Your Role Includes :

  • Providing comprehensive care for seniors with acute and chronic illnesses.
  • Engaging in collaborative care with specialists while maintaining strong oversight of your patients’ health.
  • Coordinating care for patients unable to visit the office, including those in hospitals or at home.
  • Contributing to center operations and assisting in recruitment initiatives, both for patients and other providers.

    • Qualifications :

  • MD or DO in Internal Medicine, Family Medicine, or Geriatrics.
  • Valid State Medical License and DEA registration.
  • Board certification or eligibility in your specialty.
