Position Summary :

This role is ideal for a Physician Assistant (PA-C) seeking work-life balance, autonomy, and a deeper patient connection. You'll round at various senior communities and play a vital role in delivering coordinated, relationship-based care .

Responsibilities :

• Visit patients in assisted living and memory care settings
• Provide physical exams, manage ongoing conditions, and adjust care plans
• Collaborate with physicians and support staff to optimize treatment
• Educate families and facility teams on care coordination
