About The Position

📍 On-Site | Flexible Schedule | No Nights or Weekends

💰 $60,000 – $120,000 annually (Part-Time, Based on Experience)

🔍 What You’ll Do :

Perform comprehensive health assessments and exams

Develop and manage personalized care plans

Monitor chronic and acute conditions

Order and interpret labs, diagnostics, and prescribe medications

Educate residents and families on wellness and disease management

Participate in quality improvement and care coordination initiatives

Document all care in EMR system

🗓️ Schedule :

Preferred : Monday, Wednesday, Friday (9 : 00 AM – 2 : 00 PM)

Flexible : Open to alternate part-time availability

On-Site Only

Job Requirements

📋 Requirements :

Master’s Degree in Nursing from an accredited NP program

Active Nurse Practitioner license (state-specific)

DEA registration for prescribing

Eligible for Medicare / Medicaid billing

Minimum 3 years of clinical experience (geriatric or LTC preferred)
