Internal Medicine - Geriatrics Physician

We re hiring a dedicated physician to join a respected post-acute care practice in the Danbury, CT area. This full-time position offers schedule autonomy, excellent compensation, and the opportunity to work with a nationally recognized group.

Position Details :

Location : Danbury, CT

Setting : Post-acute care (skilled nursing and assisted living facilities)

Schedule : Full-time, weekdays only no primary night call

Patient Volume : patients / day

EMR : gEHRiMed

Key Responsibilities :

Deliver primary care to patients in post-acute care communities

Diagnose and manage chronic and acute conditions

Collaborate with interdisciplinary care teams, hospice, palliative, and home health services

Ensure compliant documentation and timely billing within Medicare guidelines

Contribute to transitions of care planning and patient-centered outcomes

Qualifications :

MD or DO; Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Emergency Medicine

Active or eligible Connecticut medical license

Prior experience in post-acute care a plus, but not required training available
