Job description

Dartmouth Health is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our Outpatient Stroke Program , providing high-quality, patient-centered follow-up care for individuals recovering from stroke and other cerebrovascular conditions.

This role offers the opportunity to work alongside stroke neurologists and a multidisciplinary team to improve outcomes, prevent recurrence, and support patients in regaining their quality of life.

What Youll Do :

  • Conduct post-stroke follow-up visits, review hospital discharge plans, and monitor recovery progress
  • Manage secondary stroke prevention strategies, including medication management and lifestyle counseling
  • Order and interpret imaging, lab work, and other diagnostics as needed
  • Educate patients and families on stroke risk reduction and symptom recognition
  • Coordinate care with primary care providers, rehabilitation services, and other specialists
  • Document patient encounters accurately and thoroughly in the EMR
  • Qualifications :

  • Ability to obtain / maintain licensure as an NP or PA in New Hampshire
  • National certification as an NP or PA
  • At least two years of independent clinical experience
  • Neurology, vascular, or primary care experience preferred (stroke experience a plus)
  • Strong communication, organization, and patient education skills
