What Youll Do

• Serve as a Primary Care Physician for senior patients (Medicare Advantage).
• Provide preventive, chronic, and relationship-based care with an average of 1822 patients per day.
• Work with a full support team : including an RN, Medical Assistant, and Care Coordinator.
• Collaborate with specialists and hospitalists to improve outcomes and reduce hospitalizations.
• Engage patients through health coaching, education, and ongoing care coordination.
