Haven Place, Lock Haven, PA. seeking PRN Physical Therapist to join our amazing in-house rehab team!
The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.
Duties / Responsibilities :
- Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
- Assured all treatment is delivered in accordance with an established plan of care
- Provide clinical support and instruct patients, families, and caregivers
- Measures & records patient’s motor function, strength, and muscle performance
- Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
- Completes documentation in accordance with company policy and state / federal regulations
Skills :
SNF / Long term care experience preferredCurrent knowledge of treatment practicesKnowledge of Net Health Electronic Documentation System preferredAbility to manage patients with different types of personalitiesCareer Advantages :
