Position Details : Schedule : Full Time

Shift : Monday-Friday, 8am - 5pm (flexible schedule)

Location : This position conducts patient visits in the Lake County, Clermont area.

Role Overview :

The Home Health Physical Therapist is responsible for coordinating care for home health patients based on their individual needs. This role requires advanced assessment skills, decision-making capabilities, and the ability to educate patients, families, and caregivers on safe care practices. Responsibilities include evaluating patient needs, developing tailored care plans in consultation with physicians, and monitoring patient progress to achieve optimal outcomes.
