Licensed Practical Nurse (LPN / LVN)

Core Responsibilities
Resident Care & Clinical Duties
- Provide direct nursing care, including medication administration, treatments, and specimen collection.
- Monitor resident conditions, respond to changes, and communicate with physicians and families.
- Support restorative and rehabilitative programs and ensure care aligns with resident plans and preferences.
Team Supervision & Coordination
Lead and guide nursing assistants during your shift.Assign tasks, monitor performance, and ensure compliance with care standards.Participate in staff evaluations and support training and development.Documentation & Compliance
Maintain accurate and timely charting, including admissions, transfers, discharges, and incident reports.Transcribe physician orders and ensure proper medication handling and documentation.Uphold HIPAA and facility data security protocols.Safety & Infection Control
Enforce safety standards and hygiene practices.Ensure proper use of PPE and participate in fire / disaster drills.Support infection control procedures and report hazards or exposures.Care Planning & Communication
Review and update resident care plans.Ensure CNAs are informed and follow care plans.Collaborate with interdisciplinary teams to ensure continuity of care.Qualifications
✅ Education & Experience
Graduate of an accredited LPN / LVN program or nursing degree.Active, unencumbered LPN / LVN license in the state.CPR certification required.Experience with PointClickCare (PCC) preferred.✅ Skills & Traits
