Your day as a Case Manager - Project Link

As a Case Manager for Project Link, you will provide essential services to women living with substance use and co-occurring disorders, along with their children from birth through age seven who may be at risk for serious emotional disturbances. Your approach will be person-centered and holistic, focused on fostering stabilization, wellness, and long-term growth. You will guide families through prevention, intervention, support, and treatment strategies designed to enhance their physical, emotional, and spiritual well-being.

You will ensure continuity of care for clients, providing outreach, assessments, treatment planning, and crisis intervention while advocating for access to crucial resources such as housing, food, and medical services. This role requires a compassionate and strategic approach to navigating complex systems and collaborating with various service providers to achieve the best outcomes for families.

Requirements for this Case Manager - Project Link job

To succeed in this role, you should hold a bachelor's degree in Social Work, Psychology, Counseling, Sociology, or a related human services field, and have at least one year of experience providing case management to individuals with substance use disorders or co-occurring mental health conditions. You should be skilled at building relationships, communicating effectively, and empowering clients to achieve their goals. Proficiency in case documentation and management software, along with a valid Virginia Driver's License, is required. A deep understanding of treatment approaches, crisis intervention, and advocacy will allow you to navigate challenging situations and connect clients with the support they need. Empathy, adaptability, and strong organizational skills are essential as you help families achieve stability and success within our holistic care framework.

Knowledge and skills required for the position are :

  • Bachelor's degree in Human Services (Social Work, Psychology, Sociology or Counseling)
  • One (1) year experience providing case management services to individuals with a diagnosis of substance use disorder or co-occurring diagnosis of substance use disorder and mental illness.
  • Valid Virginia Driver's License.
