Minimum Qualifications :

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an APTA-accredited program.
• Credentials must be evaluated by one of the following : FCCPT, ICD, ICA, IERF, or IEC.
• Current state license in good standing with the State License Board.
