Role Overview :
As the Nurse Practitioner for our RPM / CCM service and new in-person clinic, you will play a pivotal role in leading our clinical team, managing patient care, and optimizing our programs to ensure the highest standards of care. This role includes providing direct, in-office care to patients in our Richmond clinic, while also supporting virtual and remote programs. You will oversee patient management, develop care plans, and work closely with a multidisciplinary team to drive patient engagement and improve health outcomes.
Key Responsibilities :
- Provide direct patient care in our Richmond clinic, including office visits, assessments, and follow-up appointments.
- Coordinate and manage the onboarding of new patients following acute hospital visits, ensuring a seamless transition to our care program.
- Lead the clinical delivery of RPM and CCM services, balancing in-person, remote, and virtual care.
- Develop and implement personalized care plans for patients with chronic conditions, focusing on proactive management and patient engagement.
- Monitor patient data remotely and provide timely interventions to prevent complications or hospital readmissions.
- Collaborate with our internal team, primary care providers, and specialists to ensure a seamless patient experience.
- Utilize data analytics to track performance metrics, identify trends, and continuously improve care delivery.
- Stay up-to-date with best practices, regulatory requirements, and technological advancements in chronic care, RPM, and clinic operations.
Qualifications :
Active Nurse Practitioner (NP) license in the state of Virginia.Current National Provider Identifier (NPI) and enrollment in the Medicare program with a Part B Medicare Provider Number.Minimum of 3 years of clinical experience, preferably in primary care, chronic care management, or a similar field.Experience with remote patient monitoring or telehealth is highly desirable.Strong clinical skills with the ability to provide both in-office and remote care.Excellent communication, leadership, and interpersonal skills.Proficiency in electronic health records (EHR) and remote monitoring platforms.Why Join Us?
