\nPhysical Therapist - Home Health - (PT - HH)\n

\nStartDate : ASAP \n\nPay Rate : $1534.08 - $1680.96 \n

\n2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year. Must have previous Home Health experience. Average of 5-6 home visits per day, covering all surrounding areas of MCDowell county. EMR : Cerner Roadnotes. Can expect to do Home Health admisions, ROC’s, recerts, functional assessments, supervising PTAs. One weekend of call every 4-6weeks required.\n

Work History : Should not be more than 90 days of gap in last 1 year
