Job Title : LPN Continuing Care

Job Specific Information : Subacute Rehab SKILLED NURSING FACILITY WORK.

Location / locations : SITE - Rehab & Nursing Kentridge - 4118 Kalamazoo Ave - Grand Rapids - NO FLOAT SITES

Shift Time : Nights

Hours : 1830p-0700a

Shift length : 12 hours

Weekend rotation : Every other weekend

On Call : None

Required certifications : LPN License, BLS

Scrub Color : Navy Blue

Job Summary : As a continuing care LPN with Corewell Health, providing top-notch patient care is paramount. Under the direction of an RN, mid-level Provider, or Physician, you'll be responsible for assessing, planning, implementing, and evaluating patient care.
